Accessing your Vision Benefit using Davis Vision

1	Accessing your Benefit is easy and simple. To access your vision benefit, first select a participating Davis Vision provider from the provider directory.   Available by: • Contacting the 32BJ Member Services Center at 1-800-603-5633. • Accessing the Find a Provider section on the 32BJ website.
2	Visit the selected provider. When calling for an appointment or visiting the provider: • Identify yourself as an Davis Vision member and a participant in the 32BJ Health Fund. • Provider your Member ID number on your Davis Vision ID card, name and date of birth, and do the same for your covered dependents seeking vision services.
3	Visit the provider office and receive an eye examination.
4	Order any needed glasses or contacts.
5	Your order is shipped to the provider - usually in five business days.
6	You can then return to the provider office for dispensing and fitting.
